Spindle-shaped fibroblasts, permeation of collagen with lymphocytes, fibrosis, granulomatous inflammation, lymphoid hyperplasia, intraalveolar fibrosis

Brief Discussions

Synonyms : plasma cell granuloma, inflammatory myofibroblastic tumor, histiocytoma, xanthoma, fibroxanthoma..

Uncommon cause of the SPN : 0.7% of lung tumor

Generally regarded as a reactive process

    - May mimic neoplasia : vertebral destruction, vascular invasion, recurrence, metastatic spread

Relationship between inflammatory pseudotumor and malignancy is controversial

Solitary, peripheral, sharply circumscribed, lobulated mass with lower lobe predilection

CT : Well-marginated and lobulated mass, heterogeneous attenuation with variable patterns of contrast enhancement and calcification (15 cases among the 61 pts : 25%)

MRI : Intermediate SI on T1WI and high SI on T2WI with variable enhancement
