Diagnosis With Brief Discussion

Discussion
Simple eosinophilic pneumonia
= Loffler's syndrome

-Blood eosinophilia, absent or mild Sx
-Nonsegmental pulmonary consolidation ?transitory and/or migratory
-Spontaneous clearing of consolidation ?generally within a month

Acute eosinophilic pneumonia
-Acute febrile illness < 5 day's duration
-Hypoxemic respiratory failure
-Alveolar or alveolar/interstitial chest radiographic opacity
-BAL eosinophil level > 25%
-Prompt & complete response to steroids without relapse
-Absence of parasitic, fungal, and other infection

Chronic esosinophilic pneumonic
-Common Hx of atopia
-Adult onset asthma
-Homogenous alveolar lung infiltrate with distribution at lung periphery; Photographic negative of pulmonary edema
-Unchanged for many days/weeks
-Dramatic response to steroid
